V. DOCUMENTS REQUIRED ARE:

a. One (1) original and two (2) copies of the “Application for Citizenship” form

b. A photocopy of current and previous travel documents such as passports or identity cards for all those applying for citizenship

c. Birth certificates of ALL those applying for citizenship. Birth certificates of those under 18 years must be issued and sealed by the proper authorities, along with the proper English translation (if necessary.) Each birth certificate should clearly show the child’s name, parents’ names place and date of birth. Birth certificates with unnamed children must be substantiated with other documentary evidence

d. Marriage certificate duly signed and sealed by the proper authority, if the spouse is also applying for citizenship

e. Copies of divorce or death certificate and the custody certificate of any children from a previous marriage, if the applicant (divorce or widower/widow) is including the children in his/her application

f. Medical certificates from a medical doctor confirming that the applicant and all those
applying for citizenship are free of all infectious diseases, including HIV.

g. Certificate of No criminal record from the competent local or government policing
authority for ALL applicants plus any other documents mentioned on page three (3)
of the “Applicant for Citizenship” form

VI. ALL PHOTOCOPIES OF ANY DOCUMENTS SUBMITTED MUST BE NOTARISED
AND PROPERLY CERTIFIED.

a. The “Applicant for Citizenship” form should be completed be EACH applicant over eighteen (18) years and submitted along with the documents identified on page three of the form.

b. The “Applicant for a Passport” form ‘A’ should be completed by EACH applicant
over sixteen (16) years of age. Every applicant should place his/her specimen signature with the date on the dotted lines in Section 7, as well as on the three dotted lines at the bottom of page 3.

c. The “Applicant for a Passport” form ‘B’ should be used for children under sixteen (16) years of age.

d. One (1) passport photograph MUST be endorsed on the reverse side by the applicant

e. If the applicant in Section 8 of the passport application form does not reside in the Federation, then such applicant should place a seal or stamp after the signature showing his/her name. If there is no seal or stamp then he/she must submit an affidavit sworn before a notary public confirming his/her position and identity. The applicant’s full name MUST be written in BLOCK CAPITALS on the line below the signature.

f. The applicant must NOT write on the back of the “Application for a Passport” form ‘A’

VII. Applications are processed in the shortest possible time.

Note: Application forms must be obtained from the Ministry of National Security.
