a. One (1) original and two (2) copies of the “Application for Citizenship” form
b. A photocopy of current and previous travel documents such as passports or identity cards for all those applying for citizenship
c. Birth certificates of ALL those applying for citizenship. Birth certificates of those under 18 years must be issued and sealed by the proper authorities, along with the proper English translation (if necessary.) Each birth certificate should clearly show the child’s name, parents’ names place and date of birth. Birth certificates with unnamed children must be substantiated with other documentary evidence
d. Marriage certificate duly signed and sealed by the proper authority, if the spouse is also applying for citizenship
e. Copies of divorce or death certificate and the custody certificate of any children from a previous marriage, if the applicant (divorce or widower/widow) is including the children in his/her application
f. Medical certificates from a medical doctor confirming that the applicant and all those
applying for citizenship are free of all infectious diseases, including HIV.
g. Certificate of No criminal record from the competent local or government policing
authority for ALL applicants plus any other documents mentioned on page three (3)
of the “Applicant for Citizenship” form
